How to use the LHS Referral Form
Our referral form is a Microsoft Office Word document (.doc).
The referral form document contains macros (that make the
selection of choices easier). Macros will not run if your
Office Word security setting is set to "High" or "Very
High". You must have security settings of "Medium" or
less to open the form.
Use the following instructions to set your security to
"Medium" (recommended). click

Please follow the recommended
procedure:-

Click the "LHS
Referral Form" hyperlink on the "Referral" page. |
| 2. |
Ensure that you select
"Save" on any "File Download" windows. (Do not use
any "Open" options that may be available) |
| 3. |
Use the "Save As"
window to save the "LHS Referral Form.doc" to your
PC's filing system. Please note that on the
latest Windows XP systems that this file might be
blocked as it has been downloaded from a website. In
Windows Explorer right mouse click in the file that
you have just downloaded and select "Properties"
from the menu:-

If the file shows as "Blocked" then hit
the "Unblock" button then the "Apply" or
"OK" button and the file will function as
intended |

Open the form from the
location that you saved it in the previous step. |

Depending on your
Microsoft Office security settings you may have to
acknowledge that you wish to open a document that is
published by "Lifeworks Health Services Pty Ltd".
The document contains macros and active content that
has been digitally signed (via a Trust Certificate)
by LHS.. Please ensure that Enable Macros is
clicked. (see below) |

To enter the information required
for the referral; place your cursor in the
relevant field and type.
Please mark any check box to indicate your selection when you are given a choice. Use your mouse to click on the box
that you wish to check.
Note that some choices have been pre-selected to defaults; so please ensure that you review these to reflect your situation.
If you are unsure of the option to select; or it is
not applicable to your situation then select the
"N/A" - Not Applicable option. |

When you have completed the referral form;
please ensure you save any changes to file. |

The referral can then be e-mailed to us as a file attachment. Alternatively, you may fax the form and any attachments to us
at the number listed on the form. |
Please repeat the process above
for any subsequent referrals.
