Lifeworks Health Services Pty Ltd is committed to providing high quality rehabilitation services. We understand that the privacy of our clients is an essential part of our services. This is to help you understand how LHS deals with the health information we collect.

Why do we collect information? It is necessary to collect our clients' health information to provide our services. To whom may we disclose the information? We may discuss our clients' health information only with any authorised person or body directly associated with their case. This may include their nominated treating doctor, their employer's nominated representative, the Insurer, other specialists (eg Physiotherapists, Vocational Counselors).

We will not disclose the client's health information to any person not directly involved in their case without the client's express consent (eg family members).

What happens to our clients' health information? Information about all clients is confidential. All client records are securely stored with LHS. All documents are accessible only by authorised staff with a need to know about a particular client.

How can clients access the information LHS holds about them?

Clients can access the health information that LHS holds about them by contacting the LHS head office in Chatswood and lodging a request. LHS will provide access to health information except to the extent that providing this access would pose a serious threat to the life or health of any individual.

Client consent

We require that all clients sign the consent form relating to the collection of their health information.
